2024 / 2025 Spring Ford Youth Wrestling Policies
I hereby certify that all information entered into this system is correct. Additionally, I hereby certify that my child is in normal health and capable of safe participation in the youth sports program. If my child’s health status were to change throughout the season, I hereby acknowledge a responsibility to notify a member of the Program’s Board of Directors, in addition to that of the head coach of the team to which my child is assigned. I further certify that my child is covered by a health insurance policy.
- I assume all risks and hazards incidental to the conduct of this program and for the transportation to and from this program, and hereby waive release, absolve, indemnify and agree to hold the Spring Ford Youth Wrestling Club (SFYWC), the directors, coaches and participants for any claim arising out of injury to my child whether the result of negligence or any other cause. In the event of an emergency, I give my permission to league officials to secure emergency treatment from a licensed health care practitioner until such time as I can be notified
- Specifically regarding concussions/traumatic brain injury (TBI), I acknowledge reviewing and understanding issues related to TBI (http://www.edc.gov/concussion/sportsfindex.htr) and that SFYWC has the duty and authority to remove a player from an activity if a head injury occurs. Additionally, it is my responsibility as the parent/guardian to ensure that the player is examined by a physician following any head injury and cleared for further physical activity.
- I hereby agree that I have read, and agree to abide by the Parents’ Code of Ethics that is posted on the SFYWC website including: Pledge to provide positive support, care and encouragement for my child participating in SFYWC activities. Encourage and practice good sportsmanship by demonstrating positive support for all players, coaches and officials at all SFYWC activities. Cheer in a positive manner at all times; refrain from any negative comments toward any player, team, coach, official or fan; I am responsible for any and all persons (ex. grandparents, friends, extended relatives, siblings etc) invited or otherwise attending practices or games to see/cheer/follow my child. I will do my best to ensure that my child attends all practices and games. If my child is unable to attend any scheduled activities, I will notify the coaching staff as soon as possible. I will ensure that my child arrives for all activities on time, and with the necessary and proper equipment (wrestling shoes, socks, gym shorts,headgear, singlet etc): I hereby acknowledge that a failure to abide by these rules can result in sanctions against the offending party. and possibly their respective child/player, including but not limited to suspension and loss of gym privileges. I also acknowledge the club’s authority to uphold its rules as it sees fit. Additionally, I agree to accept any and all punishment for breaking rules without contest, and to hold harmless any league officials responsible for administration of the rules and potentially the accompanying principles.
- I hereby acknowledge the SFYWC policy on photographs and/or videos:
I hereby irrevocably authorize SFYWC to publish such photographs, videos and for streaming, with or without associated names for the purpose of promoting SFYWC or other league of associated lawful purposes; I waive the right to inspect or approve the finished product or including written or electronic copy, wherein my likeness or my child’s likeness appears.
I waive any right to royalties or other compensation arising from or related to use of the photographs, videos and/or streaming - I hereby hold harmless and release and forever discharge SFYWC from all claims, demands and causes of action which l, my, heirs, representatives, executors, administrators, or any other person acting on my behalf or on behalf of my estate may have or may have by reason of this authorization.

I ACKNOWLEDGE, AGREE, AND CERTIFY THAT I HAVE READ AND UNDERSTAND THE TERMS AND CONDITIONS HEREOF
AND AM ENTERING INTO THIS AGREEMENT VOLUNTARILY, WITHOUT ANY INDUCEMENT OR DURESS, AND WITH FULL KNOWLEDGE OF ITS LEGAL SIGNIFICANCE AND THAT I/WE INTENDIN:
2024-2025 SEASON FOR SPRING-FORD YOUTH WRESTLING CLUB PARTICIPATION WAIVER AND RELEASE
I/WE, for myself/ourselves and for and on behalf of my/our child(ren) and/or charge(s), as his/her/their parent(s) and/or guardian(s), hereby knowingly, voluntarily, and with full knowledge of the legal significance hereof, do acknowledge and agree to the following terms and conditions, in and for consideration of and exchange for my/our child(ren) and/or charge(s) being allowed to participate in the 2024-2025 SEASON FOR SPRING-FORD YOUTH WRESTLING CLUB and its related athletics/sports program, events, and activities of whatever kind or nature (hereinafter, the “PROGRAM”), as follows:
- The risk of serious injury and/or illness from the activities involved and inherent in the PROGRAM is significant, including the potential for permanent injury, including, but not limited to, paralysis and even death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does exist; and
- In addition, participation in the PROGRAM poses a risk of having contact with and being exposed to individuals who may have been exposed to, may be carriers of, and/or who have been diagnosed with one or more communicable diseases, including, but not limited to, the COVID-19 virus, the Corona Virus, SARS-2 virus, and/or other medical illnesses, conditions, diseases, maladies, viruses, bacteria, and the like (hereinafter, collectively, as “ILLNESSES”); further, I/WE acknowledge and agree that it is impossible to completely eliminate any risk that one or more persons or or children could and can be exposed to and/or could or may become infected with such ILLNESSES through participation in the PROGRAM and/or contact with and/or close proximity with/to an individual(s) who may have, been exposed to, and/or carry such ILLNESSES; and
- I/WE, FOR MYSELF/OURSELVES AND FOR OUR CHILD(REN) AND/OR CHARGE(S), DO HEREBY KNOWINGLY AND FREELY, AND WITH FULL KNOWLEDGE OF THE LEGAL SIGNIFICANCE OF THIS WAIVER AND RELEASE, ASSUME ANY AND ALL RISKS, WHETHER KNOWN OR UNKNOWN, RELATING TO, ARISING FROM, INHERENT IN, AND/OR IN CONNECTION WITH THE PROGRAM, AND DO
HEREBY WAIVE, RELEASE, AND FOREVER DISCHARGE SPRING-FORD YOUTH WRESTLING, ITS BOARD OF DIRECTORS, OFFICERS, DIRECTORS, EMPLOYEES, AGENTS, COACHES, COMMISSIONERS, COORDINATORS, AND VOLUNTEERS (COLLECTIVELY, THE “RELEASED PARTIES”) OF AND FROM ANY AND ALL NEGLIGENCE, LIABILITY, DAMAGES, LOSSES, SUITS, LAWSUITS, COMPLAINTS, CAUSES OF ACTION, DEMANDS, JUDGMENTS, CLAIMS, EXPENSES, AND THE LIKE, OF WHATEVER KIND OR NATURE, WHATSOEVER, WHICH ARISE OR MAY ARISE FROM, RESULT OR MAY RESULT FROM, RELATE OR MAY RELATE TO, ARE BORN OR MAY BE BORN FROM, AND/OR WHICH ARE OR MAY BE CONNECTED WITH PARTICIPATION, INVOLVEMENT, AND/OR INCLUSION IN THE PROGRAM AND MY CHILD(REN)’S OR CHARGE(S)’ PARTICIPATION, INVOLVEMENT, AND/OR INCLUSION IN THE PROGRAM; and
- I/WE, for myself/ourselves and for our child(ren) and/or charge(s), do hereby willingly, voluntarily, knowingly, and with full knowledge of the legal significant hereof, agree to and consent to, and agree to comply and will comply with, any and all rules procedures, policies, guidelines, recommendations, mandates, and the like, irrespective of the particular issuing authority(ies and/or party(ies), which are or may be established, issued, recommended, implemented, and/or imposed and/or may hereafter be established, issued, recommended, implemented and/or imposed, for, in, in connection with, about, concerning, regarding, and/or relating to participation and/or inclusion in the PROGRAM; and
